By registering for services through Heart Sanctuary, you agree to the following:

• Mentorship is not a replacement for therapy, counseling, or clinical intervention.

• All information shared in mentoring sessions is treated with care and confidentiality, unless there is a concern for the mentee’s safety.

• Parents/guardians are responsible for submitting intake forms and staying informed about session times and expectations.

• The mentor reserves the right to reschedule or cancel a session in the case of an emergency or safety concern.

Heart Sanctuary reserves the right to update these terms at any time, with prior notice provided to families.
